- LICENSED THERAPIST

• Conducting initial consultations to gather patient information

• Assessing patients’ emotional and mental health developing treatment plans to meet them

• Ask questions and listening to help patients explore their thoughts and feelings

• Counseling and advising patients on issues of importance to them

• Intervening with emergency care techniques and protocols when patients are in crisis

• Providing referrals for additional care or services when required

• Maintaining detailed confidential records

• Complies with all behavioral health, state, federal, and ethical guidelines

- QBHP

• Provide behavioral health support services to clients in accordance with individualized treatment and care plans

• Assist clients with developing skills to improve daily functioning, independence, and overall well-being

• Provide case management, care coordination, and community-based support as assigned

• Assist clients in identifying and accessing healthcare, behavioral health, social, and community resources

• Monitor client progress and communicate relevant changes or concerns to the supervising mental health professional

• Work collaboratively with therapists, medical providers, case managers, families, and other members of the client's care team

• Provide services in the office, community, home, school, or other approved settings as appropriate

• Respond appropriately to behavioral health concerns and follow established procedures when clients are experiencing a crisis

• Maintaining detailed confidential records
